You'll have to play the game in this pattern:

4 stages → SPEED UP → 4 stages → SPEED UP → 4 stages → 1 boss stage → LEVEL UP →
4 stages → SPEED UP → 4 stages → SPEED UP → 4 stages → 1 boss stage → LEVEL UP →
4 stages → SPEED UP → 4 stages → SPEED UP → 4 stages → 1 boss stage → SPEED UP ↻ (Go back to the beggining of the line)

The boss stage will appear when the stage number is a multiple of 13.
(That's kind of annoying, because scoring for even 100 points isn't difficult, meaning you'll have to play the boss game A LOT!)
